Financial Agreement

Deposit Policy: 

The first hour deposit is collected prior to the date of surgery. This deposit reserves your surgical spot for the day, but also covers the first hour of anesthesia time.  Payment of the first hour deposit will be applied to the final cost of the anesthesia.  

Payment Policy:

The remaining balance is due in full at the conclusion of the dental appointment.  Example:  A 2.5 hour appointment will include the first hour deposit paid prior to the date of surgery.  The patient will be responsible for the remaining 1.5 hours of anesthesia time at the conclusion of the surgical appointment.
